Implementing Proactive Personalised Care Winter Guidance

This webinar will reflect and spotlight the work happening in PCN’s to support winter pressures, including examples of data driven approaches to target vulnerable groups and community-based interventions to address non-clinical patient needs and increase operational resilience.

The Learning Objectives are:

• Showcase PCNs that have started to use data to design new models of proactive social prescribing to deliver improvements in health and wellbeing
• Exemplars of collaborative work with communities and the voluntary sector to support people with the most complex needs
• Learn how health and care partners are working in new ways to take collective responsibility to improve access to services, meet non-clinical demand and deliver better patient outcomes
