PCN Webinar
Please join us as we continue our series of webinars focused on Primary Care Networks. The next webinar will be held on Thursday 26th January from 5pm-6.00pm and will cover implementing proactive personalised care to support primary care pressures.
During this webinar, we will showcase the three personalised care roles, using case studies from around the country to share examples of good practice, and identify current challenges that you may be facing and suggest potential solutions to overcome these.
