DESCRIPTION:L&SC ICB Population Health team are hosting a best practice session on 6th Feb (12:00-13:00). \nICB Health Inequality Clinical Lead and local Burnley GP\, Dr Lucy Astle will be talking about the population health approach taken by the two Burnley PCNs to provide a more inclusive health offer. Lucy and colleagues were keen to offer enhanced health checks to underserved and potentially excluded cohorts of patients to better understand community need. The team worked with the community and partner agencies to engage\, collaborate and co-produce outreach services in accessible locations and in doing so built new and trusted relationships in the area. Book here  \nEveryone is welcome that has an interest in population health\, reducing health inequalities and working to improve health outcomes and reduce inequity.

DESCRIPTION:Available to Social Care staff working within the Lancashire and South Cumbria postcodes only: \nSession Aims: \nThe purpose of this session is to develop the skills and knowledge of HCA’s in the correct use of medication\, how to ensure it is safely managed stored\, administered and recorded effectively. \nLearning Outcomes: \n  \n• How to administer medicines in a systematic way to reduce the risk of errors \n• How to give medicines in a person-centred way \n• Explain why mistakes happen and how to minimise the chances of them occurring \n• How to administer tablets\, liquids\, creams\, inhalers\, patches\, and eye drops \n• How to record when medication is administered to an individual using service specific documentation here possible \n  \nMethod of learning Microsoft Teams 3 hours \nThe training is entirely online learning which is led by a tutor in real time. These virtual sessions are a mixture of group work\, discussions\, practical activities as well as space for individual reflections and breaks to main energy.
