DESCRIPTION:This webinar will support colleagues in primary care to understand how to implement personalised proactive care\, using a targeted approach to reduce reliance on clinical input through improved biopsychosocial support. It will explain how to make best use of the skills and expertise of social prescribing link workers\, health and wellbeing coaches and care co-ordinators to tackle population health needs. DESCRIPTION:In this webinar colleagues from Tameside and Glossop Integrated Care NHS Foundation Trust will be showcasing best practice in terms of their Make Every Contact Count (MECC) and Transition to Practice interventions for pre-registered learners. \n\nMake Every Contact Count (MECC) enables the delivery of concise mental health and wellbeing information and encourages individuals to be more confident to engage in conversations about their health and the health of others. The MECC approach recognises that staff who have significant contact with others are ideally placed to support their own health and wellbeing as well as that of other individuals.\nTransition to Practice sessions allow an easier transition from being a learner to becoming a qualified member of staff.
