SUMMARY:PAM - Patient Activation Measure 2.5hr Online
DESCRIPTION:The PAM® can make a valuable contribution to assessing a patient’s ability to self care at any given time. This assessment helps support next steps on what would be needed to increase their levels of knowledge\, skills and confidence in order to improve their health and wellbeing outcomes. It can be used to tailor interventions to individual needs\, significantly increasing the likelihood that people will adopt behaviours that contribute to better health. It can also help commissioners to make available the kinds of interventions needed\, especially for those with low levels of activation who have most to gain. \nThough this course is offered to you fully funded\, please make every effort to attend once you have enrolled\, or make us aware so that someone else may benefit. Thank you!

SUMMARY:Mental Health in Primary Care
DESCRIPTION:This session is designed for anyone working clinically in primary care who encounters mental health presentations as part of everyday practice. \nMental health difficulties are common\, complex\, and often intertwined with physical health\, social stressors\, and life events. In primary care\, we are uniquely placed to notice early signs\, open meaningful conversations\, and offer timely\, compassionate support. \nUsing a primary care–relevant case study\, this session will focus on normalising mental health\, exploring how distress commonly presents in primary care settings\, and introducing formulation as a practical framework for making sense of people’s difficulties beyond diagnosis alone. \nThe session will introduce the 5 Ps formulation model (presenting\, predisposing\, precipitating\, perpetuating and protective factors) as a shared language for understanding mental health problems collaboratively with patients. We will consider how formulation can guide clinical decision-making\, risk awareness\, and proportionate intervention in a time-limited primary care context. \nParticipants will explore realistic support options within primary care\, including brief interventions\, psychoeducation\, social prescribing\, medication considerations\, referral pathways\, and safety-netting. The emphasis throughout is on confidence\, curiosity\, and relational practice rather than “fixing” or over-pathologising distress. \nThis session aims to support clinicians to feel more confident having mental health conversations\, to think holistically\, and to recognise the valuable role primary care plays in mental health support. \nThis is an interactive session and you will be expected to interact. Please ensure you have a camera and microphone available to attend the meeting. \nYou will receive a certificate post training for your attendance. Places are limited\, if you have booked a place\, please ensure you attend.\nIf you have any additional needs or requirements to attend this session\, please contact us at mbpcc.lscthub@nhs.net\nBy signing up you agree for your information to be stored and shared by the Training Hub and with the Training
