DESCRIPTION:The PAM® can make a valuable contribution to assessing a patient’s ability to self care at any given time. This assessment helps support next steps on what would be needed to increase their levels of knowledge\, skills and confidence in order to improve their health and wellbeing outcomes. It can be used to tailor interventions to individual needs\, significantly increasing the likelihood that people will adopt behaviours that contribute to better health. It can also help commissioners to make available the kinds of interventions needed\, especially for those with low levels of activation who have most to gain. \n  \nThough this course is offered to you fully funded\, please make every effort to attend once you have enrolled\, or make us aware so that someone else may benefit.
