SUMMARY:Best Practice - Population Health Respiratory Project
DESCRIPTION:Naomi will be talking about the population health approach taken in Central Lancashire for residents living with respiratory conditions such as COPD and asthma. \nNaomi will demonstrate how the Respiratory Resilience Over Winter project has helped to support our most vulnerable residents with respiratory conditions\, helping them to manage their conditions; supporting them through clinical as well as social prescribing interventions to assess the deeper determinates of health\, and enabling them to stay well over winter. \nEveryone is welcome that has an interest in population health\, COPD and CORE20PLUS5 and working to improve health outcomes and reduce inequity. \nPlease join us for what promises to be both an interesting but also much needed session on this important topic as now is the time to start making plans for winter respiratory care.
