DESCRIPTION:June – Enhanced Care Homes & Frailty  \nDES: Care Homes\, anticipatory care  \nLES: Enhanced Care Homes Service  \n\nWeekly home rounds and MDT working \n\n\nFrailty identification and care planning \n\n\nAdmission avoidance and escalation pathways \n\nLancashire and South Cumbria Primary Care Training Hub’s PCN Lead Nurse Forums are held monthly for 30 minutes \nThe Forum provides useful resources\, regular update of communications from local Integrated Care Board such as Infection Control and Safeguarding and National Steer. It allows nurses to share best practice and innovation across the system\, collectively raise any nurse issues and provides peer support. \n\nAs part of the General Practice Nurse Programme in Lancashire and South Cumbria\, we have funding for a Lead Nurse in Primary Care Networks to join our collaborative. The collaborative is continuing the work from the GPN10PP to collectively work together to support all nurses in Primary Care.\n​\n​This is an interactive session and you will be expected to interact.
