DESCRIPTION:Who should attend:\nAny staff member who is involved in and/or has an interest in supporting people with learning disabilities (PWLD) to have annual health checks. \n\nDescription of training:\n\nTraining sessions will cover what a learning disability is/is not\, and the characteristics seen within different levels of LD\nOverview of LD inclusion tool to identify patients\, when and how to use\nOverview of LeDeR – purpose and statistics of preventable deaths in PWLD\nImportance of Annual Health Checks and common health issues in PWLD\nMental Capacity in relation to healthcare for PWLD\nBarriers to accessing equitable healthcare for PWLD\nAHC and health action plans – what should be considered\, how to develop an effective HAP\nEffective strategies to support successful AHC’s\nService aims and objectives of the Health Facilitation Team to support GP practices\nLearning Disability Champion proposal\nVideo’s from PWLD – explanation of their experiences of health checks\n\n\n\nThis is an interactive session and you will be expected to interact. SUMMARY:PCN Lead Nurse Forum
DESCRIPTION:We host a Virtual Forum for PCN Lead Nurses/Representatives\, once a month (day and time changes monthly to allow accessibility to all). \nThe Forum provides useful resources\, regular update of communications from local Integrated Care Board such as Infection Control and Safeguarding and National Steer. It allows nurses to share best practice and innovation across the system\, collectively raise any nurse issues and provides peer support. \n\nAs part of the General Practice Nurse Programme in Lancashire and South Cumbria\, we have funding for a Lead Nurse in Primary Care Networks to join our collaborative. The collaborative is continuing the work from the GPN10PP to collectively work together to support all nurses in Primary Care.
