DESCRIPTION:April – Setting the Direction \nDES: Overall delivery\, workforce\, access \nLES: Transformation priorities \nPCN DES 2026–27 requirements overview \nLocal LES priorities for Lancashire & South Cumbria \nAgree nursing KPIs: access\, care quality\, anticipatory care \n  \nLancashire and South Cumbria Primary Care Training Hub’s PCN Lead Nurse Forums are held monthly for 30 minutes \nThe Forum provides useful resources\, regular update of communications from local Integrated Care Board such as Infection Control and Safeguarding and National Steer. It allows nurses to share best practice and innovation across the system\, collectively raise any nurse issues and provides peer support. \nAs part of the General Practice Nurse Programme in Lancashire and South Cumbria\, we have funding for a Lead Nurse in Primary Care Networks to join our collaborative. The collaborative is continuing the work from the GPN10PP to collectively work together to support all nurses in Primary Care.\n​\n​This is an interactive session and you will be expected to interact. SUMMARY:Mental Health in Primary Care
DESCRIPTION:This session is designed for anyone working clinically in primary care who encounters mental health presentations as part of everyday practice. \nMental health difficulties are common\, complex\, and often intertwined with physical health\, social stressors\, and life events. In primary care\, we are uniquely placed to notice early signs\, open meaningful conversations\, and offer timely\, compassionate support. \nUsing a primary care–relevant case study\, this session will focus on normalising mental health\, exploring how distress commonly presents in primary care settings\, and introducing formulation as a practical framework for making sense of people’s difficulties beyond diagnosis alone. \nThe session will introduce the 5 Ps formulation model (presenting\, predisposing\, precipitating\, perpetuating and protective factors) as a shared language for understanding mental health problems collaboratively with patients. We will consider how formulation can guide clinical decision-making\, risk awareness\, and proportionate intervention in a time-limited primary care context. \nParticipants will explore realistic support options within primary care\, including brief interventions\, psychoeducation\, social prescribing\, medication considerations\, referral pathways\, and safety-netting. The emphasis throughout is on confidence\, curiosity\, and relational practice rather than “fixing” or over-pathologising distress. \nThis session aims to support clinicians to feel more confident having mental health conversations\, to think holistically\, and to recognise the valuable role primary care plays in mental health support. \nThis is an interactive session and you will be expected to interact.
