DESCRIPTION:This webinar will support colleagues in primary care to understand how to implement personalised proactive care\, using a targeted approach to reduce reliance on clinical input through improved biopsychosocial support. It will explain how to make best use of the skills and expertise of social prescribing link workers\, health and wellbeing coaches and care co-ordinators to tackle population health needs.
