CARE Programme National Event – Central Birmingham
NHS England and the National Association of Primary Care (NAPC) are inviting you to a national event to connect, share and be inspired by exemplars of practice that were designed and delivered by health and care staff across the country.
The event will showcase how CARE – a NHS England and NAPC development programme that creates a culture of learning and innovation – has empowered healthcare professionals to deliver population health improvements within their communities and shape neighbourhood care through integrated working.
The sessions will include real-life examples of population health improvement projects in neighbourhood settings that are both cost effective and make a difference to people’s lives, which also have the system wide benefits of reducing demand on services and improving workforce wellbeing. There will be a strong focus on evidence and ‘how to’ to help attendees replicate good practice in their system to support transformational change.
Discussion will also focus on the outcomes and evidence of how effective integrated neighbourhood teams can deliver proactive personalised care that meets the identified health and wellbeing needs of the population, in line with the Fuller stocktake. Plus, there’ll be plenty of opportunities to meet and share insights with peers and leaders through protected networking sessions.
