Social prescribing services across Northeast, Yorkshire & Humber will provide insights into recent innovations in delivery models. Through ongoing activities to codesign social prescribing services with local authority, VCSE, PCNs and patients across the region, we are progressively improving access to a social model of health.

While social prescribing continues to expand in size and scale, it also continues to deliver on the fundamentals of supporting patients to discuss ‘what matters to me’.
